We may revise these Terms at any time. Revisions are effective on posting. Continued use of our services after any revision means you accept the updated Terms.

These Terms of Use and Patient Service Agreement ("Terms") set out the rules that apply to every interaction you have with CareVibe Medical Clinic Inc. — visiting our website, creating a patient account, booking appointments, submitting forms, communicating with our team, and receiving clinical care through our platform. Read them in full. If you do not agree with any part, do not use our website or services.

1. Who We Are

CareVibe Medical Clinic Inc. is a virtual healthcare that connect patients with regulated Nurse Practitioners (NPs) and other members of the team like therapists or clinicians where applicable who deliver care.

Every NP who provides services through CareVibe is individually licensed by their provincial regulatory college, and is personally and professionally accountable for the clinical care they deliver. CareVibe as a corporation does not direct or control clinical judgment. That authority belongs exclusively to your NP.

CareVibe currently operates in Ontario and British Columbia. As we grow, additional regulated NPs and potentially other health professionals may join the platform in provinces where CareVibe has obtained the required regulatory authorizations. These Terms are written to remain valid as that expansion happens.

Your therapeutic and legal relationship for clinical care is with your Nurse Practitioner — not with the corporation. Each NP on the CareVibe platform is independently registered, professionally insured, and answerable to their provincial college for every clinical decision they make.


CareVibe is not a hospital, emergency service, crisis service, or walk-in clinic.

Description of Services:
-The patient can get an understanding of what services are provided on our website and choose to register for a virtual appointment to see a Nurse Practitioner. Services may include:
1. ADHD assessments
2. Ongoing ADHD follow up care (which may include medication management)
3. Completing a self-administered 9 question questionnaire to see if you might benefit from an ADHD assessment.

2. Legislation

⚠ CAREVIBE IS NOT AN EMERGENCY SERVICE.
If you are in a medical emergency of any kind — including but not limited to chest pain, dizziness, difficulty breathing, overdose, stroke symptoms, active suicidal intent, or severe psychiatric crisis — call 911 or go to your nearest emergency room immediately. Do not message us. Do not email. Do not wait. CareVibe has no 24/7 clinical monitoring capability and cannot respond to emergencies. For around-the-clock mental health crisis support, call or text 9-8-8 (Suicide Crisis Helpline).

3. Who Can Use CareVibe

To access clinical services through CareVibe you must meet all of the following criteria at the time of each and every appointment:
  • You are 18 years of age or older
  • You are physically located in a province or territory where CareVibe holds required regulatory registration at the time care is delivered — currently Ontario and British Columbia only
  • You have provided truthful, complete, and current identity and health information
  • You have legal capacity to consent to healthcare on your own behalf
  • You have completed all required consent and intake documents before the appointment begins

We may ask you to confirm your physical location at any point before or during an appointment. If you are outside a province where we are authorized to practice, care cannot proceed and you will not be charged.

Browsing this website or submitting a contact form does not make you a CareVibe patient and creates no clinical relationship. A patient relationship begins only once you have booked an appointment, completed all required consent documents, and attended your first clinical appointment.


CareVibe reserves the right to decline registration or access to services at our discretion, provided we do not do so on prohibited grounds under applicable human rights legislation.

4. Acting on Behalf of Another Person

CareVibe's clinical services are for adults 18 and older accessing their own care. If you are a legal guardian, substitute decision-maker, or authorized representative acting on behalf of an adult who lacks capacity to consent independently, you must:
  • Confirm your legal authority to make healthcare decisions for that person before booking
  • Provide truthful and complete information on their behalf throughout all clinical encounters
  • Accept that all obligations in these Terms apply to you as their authorized representative

CareVibe does not currently provide services to minors. You may not access services on behalf of another adult without lawful authority to do so. Doing so without authorization constitutes a breach of these Terms and may constitute fraud.

5. When CareVibe Is Not the Right Fit

CareVibe specializes in virtual ADHD assessment and medication management for adults with mild to moderate presentations. Because all of our care is delivered through a screen, there are clinical situations we cannot safely serve. If you present with any of the following, our NPs will refer you to a more appropriate provider and will not proceed with services:
  • Active suicidal ideation requiring immediate safety intervention
  • Active substance use disorder requiring structured addiction treatment
  • Bipolar disorder requiring specialist psychiatric oversight
  • Eating disorders requiring a specialized multidisciplinary care team
  • Post-Traumatic Stress Disorder (PTSD) requiring trauma-focused specialist care
  • Obsessive Compulsive Disorder (OCD) requiring specialist psychological or psychiatric treatment
  • Gender dysphoria requiring specialized gender-affirming healthcare
  • Traumatic Brain Injury with active symptoms or ongoing specialized treatment
  • Cognitive impairment that makes standard ADHD assessment clinically unreliable or inappropriate
  • Current or historical physical abuse where specialized safety planning is required alongside clinical care

Being declined care on this basis means you will not be charged for an appointment that does not proceed. We will provide referral information. This list reflects our current scope and will be updated as our clinical team and service capacity grows.

6. What Virtual Care Can and Cannot Do

All care at CareVibe is delivered via secure video through Juvonno — a Canadian-hosted, end-to-end encrypted, PHIPA-compliant platform. By using our services you agree to receive clinical care through this virtual format. You can withdraw this consent at any time — though doing so may mean we can no longer safely provide your care, and we will assist you in transitioning to another provider.

Virtual care has genuine benefits and genuine limitations. Before you book, understand both:
  • We cannot physically examine you. If a physical exam is necessary for safe assessment or treatment, you will be referred to in-person care.
  • Technology can fail. Poor video or audio quality, dropped connections, or platform outages may affect your appointment. Where a failure is on our side, you will not be charged.
  • Our assessment relies on your self-reported history, validated clinical rating scales, and where appropriate, input from someone who knows you well. We do not conduct neuropsychological testing at this time.
  • Your NP makes a fresh assessment at each appointment about whether virtual care remains appropriate for your situation. A booking confirmation does not override that clinical judgment.
  • Privacy at your location is your responsibility. Attending from a shared or public space where others can overhear is at your own risk. CareVibe recommends a private location and headphones.

7. What We Expect From You

Safe and effective virtual care depends on honest, responsible participation from every patient. By using CareVibe you agree to:
  • Provide complete, accurate, and current information about your health history, all medications and supplements, allergies, substance use, prior psychiatric diagnoses and treatments, and any current safety concerns — at every visit
  • Attend appointments from a private location where you can speak freely and confidentially
  • Share your current physical location and emergency contact information when asked
  • Follow through on clinical recommendations and seek in-person or emergency care when your NP advises it
  • Use our patient portal and communication channels for their intended clinical and administrative purpose only
  • Protect your account login credentials and never share them with anyone else
  • Treat our NPs and every member of our team with respect in every interaction, through every channel
  • Notify CareVibe promptly if your province of residence changes or if you will be located outside Ontario or British Columbia at the time of your appointment

8. Feedback and Accessibility Complaints

The following conduct is strictly prohibited and will result in immediate and permanent termination of access to CareVibe services:
  • Providing false or misleading information for the purpose of obtaining a diagnosis, prescription, or clinical service of any kind
  • Seeking to obtain controlled substances fraudulently — this is a criminal offence under the Controlled Drugs and Substances Act (Canada)
  • Attempting to access services for another person without lawful authority to do so
  • Sharing, selling, transferring, or misusing any prescription issued through CareVibe
  • Attempting to access, scrape, reverse-engineer, copy, or interfere with any part of our platform or systems
  • Introducing malicious code or attempting to compromise the security of our systems or the PHI of other patients
  • Impersonating another person or submitting information under a false identity
  • Using CareVibe systems for any unlawful purpose under Canadian law
  • Recording any appointment without prior written consent from your NP
  • Any conduct that interferes with the safe and effective delivery of care to other patients

9. Multi-Year Accessibility Plan

A confirmed booking and a paid fee do not guarantee any prescription. Every prescribing decision is made solely on clinical grounds by your NP, based on your individual presentation, the information available, applicable law, and professional standards. CareVibe does not and cannot guarantee prescriptions of any kind.

Stimulant medications — including but not limited to Vyvanse, Adderall XR, Biphentin, and Concerta — are controlled substances under Canadian federal law. They carry clinical risks and are subject to strict legal obligations. At CareVibe:
  • No stimulant or other controlled substance is prescribed at the initial ADHD assessment appointment — this is a fixed clinical safety policy that cannot be waived
  • Before initiating any controlled substance, your NP may require additional information including laboratory results, cardiovascular screening, prior medical records, collateral history, or specialist consultation
  • CareVibe operates a one-prescriber, one-pharmacy model — all prescriptions are sent electronically directly to your designated pharmacy
  • Every prescription is for your personal use only — you may not share, sell, transfer, or misuse any medication prescribed through CareVibe
  • Your NP may at any time modify, taper, decline to renew, or discontinue any medication based on clinical judgment, new information, or safety concerns — a prior prescription does not guarantee a future one

⚠ For severe side effects, allergic reactions, overdose, serious withdrawal, new suicidal thoughts, chest pain, difficulty breathing, fainting, mania, or psychosis — get emergency care immediately. Call 911 or go to your nearest emergency room. Do not send CareVibe a message and wait for a response, seek immediate care at your nearest emergency department or call 911.

10. Fees, Billing, and Cancellations

CareVibe is a private-pay clinic. No services are billed to OHIP, BC MSP, or any provincial health plan. All fees are disclosed before you book and are governed in full by our Fee Schedule & Cancellation Policy (ADM-001), which forms part of your agreement with CareVibe and must be acknowledged before your first appointment.
  • Initial ADHD assessment: $449 CAD — flat fee, no hidden charges
  • Follow-up appointments: $69 each
  • When your card is charged: After your appointment is completed — not at booking. Your credit card is stored securely via Juvonno using Stripe (PCI-DSS compliant). CareVibe does not store raw card data.
  • Cancellations — same-day or no-show: 100% of the appointment fee. No refund.
  • Cancellations — 24 to 48 hours notice: 50% charge. 50% refund.
  • Cancellations — 48+ hours notice: Full refund.
  • Completed appointments: Non-refundable. If you have a concern, contact us — we will respond in good faith.
  • Chargebacks: Filing a chargeback for a legitimately delivered service constitutes a breach of these Terms. CareVibe will dispute chargebacks for valid completed appointments and may suspend access to non-urgent services during any payment dispute.
  • Reimbursement: Your fees may qualify for extended health benefits, a Health Spending Account (HSA), or the federal Medical Expense Tax Credit. Verify with your plan provider. CareVibe cannot guarantee or facilitate any third-party reimbursement.

11. How We Communicate With You

CareVibe may contact you by email, phone, patient portal, secure messaging, video platform, SMS, e-fax, or mail for appointment-related and administrative purposes. Clinical communications happen through your secure Juvonno portal.
Do not use any CareVibe communication channel for urgent or emergency matters. We do not monitor messages in real time and cannot respond to medical emergencies through our portal or email. For anything urgent, call 911 or go to your nearest emergency room.

You are responsible for keeping your contact information current in Juvonno. We cannot be held responsible for missed communications due to outdated contact details.

Any marketing or promotional communications CareVibe sends will comply fully with Canada's Anti-Spam Legislation (CASL):
  • Commercial electronic messages are sent only with your express or implied consent
  • Every commercial message clearly identifies CareVibe and includes our contact information
  • Every commercial message includes a working unsubscribe mechanism
  • We honor all unsubscribe requests within 10 business days

Clinical and transactional communications — appointment confirmations, clinical correspondence, and receipts — are not subject to CASL's commercial messaging requirements but remain governed by our privacy obligations.

12. Your Health Information and Privacy

Our complete Privacy Policy is published at www.adhdcare.ca/privacy and is incorporated into these Terms by reference. Key points:
  • Your personal health information (PHI) is protected under PHIPA (Ontario), the E-Health Act (BC), and equivalent legislation in any additional province where CareVibe expands
  • All records are stored in Juvonno on Canadian servers, encrypted at rest and in transit, accessible only to authorized clinical staff involved directly in your care
  • CareVibe will never sell, rent, share, or monetize your PHI for any commercial purpose
  • We may disclose your information without your consent only where Canadian law expressly requires it — for example mandatory child protection reporting, public health reporting, or a valid court order
  • You have the right to access your own health records and to request correction of inaccurate information — contact info@adhdcare.ca
  • Where CareVibe uses anonymized or de-identified data for quality improvement or research, it will do so only in compliance with applicable law and without identifying individual patients

13. Technology, AI, and Third-Party Platforms

CareVibe uses the following third-party platforms to deliver our services. Each has been selected to meet applicable Canadian privacy and security standards:
  • Juvonno — electronic medical record (EMR), patient scheduling, video visits, and patient portal (Canadian-hosted)
  • Stripe / Moneris — payment processing (PCI-DSS compliant; no raw card data stored by CareVibe)

The headings used in this agreement are included for convenience only.

CareVibe may in future introduce secure AI-assisted tools for administrative functions such as appointment documentation, transcription, or scheduling. Where any such tool interacts with PHI, we will put appropriate contractual and technical safeguards in place, update this section, and give existing patients advance notice. Clinical decisions will always remain the sole responsibility of your licensed NP — no automated or AI system will make or influence clinical judgments on this platform. CareVibe does not use identifiable PHI to train any AI model.

Third-party platforms, pharmacies, laboratories, and payment processors operate under their own terms and privacy policies. CareVibe selects platforms thoughtfully but is not responsible for services or failures of third parties we do not own or control. CareVibe is not liable for any harm or damages done from using third party platforms/links/or the like.

14. Your Account Security

Your Juvonno patient account is for your use only. You are solely responsible for all activity that takes place under your account. To protect yourself and other patients:
  • Keep your login credentials strictly private — never share your username or password with anyone
  • Do not allow any other person to access your account, view your records, or attend your appointments under your login
  • Contact us immediately at info@adhdcare.ca if you suspect unauthorized access to your account
  • Use a strong, unique password for your account

CareVibe may suspend account access if we have reasonable grounds to believe a security breach, unauthorized access, or misuse has occurred — with or without prior notice, depending on the severity of the risk to patient safety and data security.

15. Letters, Forms, and Clinical Documentation

CareVibe provides written clinical documentation where it is medically supported by your records and clinically appropriate:
  • Diagnosis confirmation letter — no charge
  • Workplace or school accommodation letter (one letter per patient) — no charge
  • Chart transfer to another provider — $50 administrative fee, waived in emergency circumstances at NP discretion

CareVibe will not complete forms or letters that are clinically unsupported, inaccurate, outside our professional scope, or inconsistent with our regulatory obligations — regardless of the purpose for which you need them. We cannot issue documentation backdated to any period before you became a CareVibe patient. Completion of documentation does not guarantee approval by any third party such as an employer, school, insurance provider, or government agency.

16. This Website Is Not Medical Advice

All content on www.adhdcare.ca is published for general informational purposes only. Nothing on this website constitutes medical advice, a clinical diagnosis, a treatment recommendation, or a substitute for assessment by a licensed health professional. Do not use website content to make decisions about your health, medication, or treatment without first speaking with a qualified provider.

CareVibe makes reasonable efforts to keep website content accurate and current, but we do not warrant its completeness or applicability to your individual situation. Content may be updated, revised, or removed at any time without notice. Any reliance you place on website content is entirely at your own risk.

17. Ownership of Content

All content on this website and within the CareVibe platform — including written content, visual design, logo, brand elements, clinical tools, intake forms, and educational materials — is the property of CareVibe Medical Clinic Inc. or our licensors and is protected under the Copyright Act (Canada) and applicable intellectual property law.

You are granted a limited, personal, non-transferable, non-exclusive licence to access and view this website for your own lawful, non-commercial use in connection with receiving or considering our services. You may not reproduce, copy, modify, distribute, publish, sell, scrape, reverse-engineer, or commercially exploit any content from this website or platform without prior written permission from CareVibe Medical Clinic Inc.

The CareVibe name, logo, and any associated brand elements are the exclusive property of CareVibe Medical Clinic Inc. They may not be used in any context without prior written consent. Your personal health information remains governed by applicable privacy legislation and is not subject to these intellectual property provisions.

18. Non-Assignment

Your rights under these Terms are personal to you and may not be assigned, transferred, or sublicensed to any other person without CareVibe's prior written consent. CareVibe may assign, transfer, or delegate any or all of its rights and obligations under these Terms — for example in the event of a corporate restructuring, acquisition, or provincial expansion — without requiring your consent, provided your rights as a patient under applicable law are not materially diminished.

19. Platform Availability

CareVibe works to maintain reliable, continuous access to our website and patient platform, but we cannot guarantee that access will always be uninterrupted or error-free. Services may be temporarily unavailable due to scheduled maintenance, unplanned technical outages, third-party platform failures, provider availability, jurisdictional regulatory requirements, force majeure events, or other circumstances within or outside our reasonable control.

Where a technical failure on CareVibe's side directly prevents your booked appointment from proceeding, we will rebook you as a priority and you will not be charged a cancellation fee. Where service disruption arises from circumstances outside our reasonable control, our liability is limited as set out in Section 22.

20. When We May Decline or End Care

CareVibe and our NPs have the right to decline to initiate or to discontinue care when doing so is clinically, ethically, legally, or operationally the right course. This includes situations where:
  • Your clinical presentation or needs exceed what virtual NP care can safely and appropriately address
  • You have provided false, misleading, or materially incomplete information about your health or identity
  • We have reasonable grounds to believe that controlled substances are being misused, diverted, or sought fraudulently
  • Your conduct toward any member of our team has been abusive, threatening, harassing, or discriminatory
  • Outstanding fees for non-urgent services remain unpaid following reasonable notice, subject to our professional obligations to patient safety
  • The therapeutic relationship between you and your NP has deteriorated to a point where continuing care is no longer clinically appropriate or safe
  • CareVibe ceases operations in a province or discontinues specific services

When ending a care relationship, CareVibe and our NPs act in accordance with CNO and BCCNM professional standards — including providing reasonable advance notice wherever possible, bridge prescriptions for controlled substances where clinically appropriate and safe, and clear referral information for alternative providers. Our detailed discharge process is governed by ADM-015 (Patient Discharge and Termination of Care Policy). CareVibe will never leave a patient in an active medical or psychiatric emergency without directing them to appropriate urgent care.

21. No Guarantees

CareVibe is committed to delivering services to a professional and reasonable standard. We cannot, however, guarantee any specific diagnosis, prescription, clinical outcome, treatment result, processing timeline, or approval by any third party. Our website content and platform tools are provided in good faith but without any express or implied warranty of accuracy, completeness, timeliness, or fitness for any particular purpose. Nothing in these Terms removes or limits any rights you hold under mandatory Canadian consumer protection, privacy, or health professions legislation.

22. Our Liability Is Limited

To the maximum extent permitted by applicable Canadian law, CareVibe Medical Clinic Inc. — including its directors, officers, employees, contractors, agents, and NPs in their capacity as platform users — shall not be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages arising from or in connection with:
  • Your access to, use of, or inability to access or use this website or the CareVibe platform
  • Any reliance on website content that falls outside a formal clinical encounter
  • Platform or technology interruptions, failures, or security incidents where CareVibe took reasonable preventive measures
  • Harm that resulted directly from inaccurate, incomplete, or false information you provided
  • Your failure to follow clinical advice or to seek emergency care when your NP recommended it
  • Acts, errors, or omissions of third-party platforms, payment processors, pharmacies, or laboratories
  • Delays, modifications, or unavailability of services caused by events outside CareVibe's reasonable control

23. Indemnification

You agree to indemnify, defend, and hold harmless CareVibe Medical Clinic Inc. and its directors, officers, employees, agents, contractors, and NPs from and against any claims, demands, proceedings, losses, damages, liabilities, costs, and expenses — including reasonable legal fees — that arise from or relate to:
  • Your breach of any provision of these Terms
  • Your misuse of the CareVibe platform, website, or clinical services
  • Any false, misleading, fraudulent, or incomplete information you provided to CareVibe or to any NP
  • Your violation of any applicable law, including criminal law, privacy legislation, or healthcare fraud provisions
  • Your infringement of the rights of any third party

24. Zero Tolerance for Abuse

Every person who works at or with CareVibe has the right to do so in a safe environment, free from abuse. The following conduct is not tolerated under any circumstances and will result in immediate termination of your access to CareVibe services — without refund and without further notice — and may be reported to law enforcement or regulatory bodies:
  • Verbal or written abuse, threats, intimidation, harassment, or discrimination directed at any CareVibe NP or staff member through any channel
  • Recording any appointment by any means without the prior written consent of your NP
  • Any fraudulent, deceptive, or manipulative conduct in connection with obtaining a clinical service, diagnosis, or prescription
  • Any conduct that materially threatens the safety, security, or integrity of our platform, our team, or our other patients

25. Severability

If any provision of these Terms is found to be invalid, illegal, void, or unenforceable by a court of competent jurisdiction, that provision will be modified to the minimum extent necessary to make it valid and enforceable. If it cannot be modified, it will be severed. The invalidity of any provision will not affect the validity or enforceability of any remaining provision, which will continue in full force and effect.

26. No Waiver

CareVibe's failure to enforce any provision of these Terms on any occasion does not constitute a waiver of that provision or of our right to enforce it on any future occasion. No waiver of any provision is effective unless it is made in writing and signed by an authorized representative of CareVibe Medical Clinic Inc.

27. The Complete Agreement

These Terms of Use, together with our Patient Privacy Notice, Fee Schedule & Cancellation Policy (ADM-001), Informed Consent for Virtual Care (ADM-002), and any other CareVibe policies expressly incorporated by reference, constitute the entire and complete agreement between you and CareVibe Medical Clinic Inc. regarding your access to and use of our website and clinical services. These Terms supersede and replace all prior and contemporaneous representations, agreements, and understandings — written or verbal — between you and CareVibe. In the event of any conflict between these Terms and any supplementary policy documents, the supplementary terms will govern with respect to the specific subject matter they address.

28. Governing Law

These Terms are governed by the laws of Ontario, British Columbia and other applicable laws in Canada.

29. How We Update These Terms

CareVibe may revise these Terms from time to time to reflect changes in our services, applicable law, technology, privacy practices, or geographic expansion. Revised Terms will be posted at www.adhdcare.ca/terms with an updated effective date.

For material changes — meaning changes that meaningfully alter your rights or obligations — we will give existing patients reasonable advance notice by email through Juvonno before the changes take effect. For non-material updates such as clarifications or minor corrections, we will post the revision without advance notice.

Continuing to use CareVibe's website or services after the effective date of any revision constitutes your acceptance of the updated Terms. If you do not agree with a material revision, stop using our services before the revised Terms take effect and notify us at info@adhdcare.ca.
